Under the name CagriSema, Novo Nordisk studies two metabolic peptides as one whole: cagrilintide, an amylin analogue, and semaglutide, a GLP-1 agonist. Which two substances are in here?
CagriSema is the name Novo Nordisk uses for the research combination of cagrilintide and semaglutide. The first is an amylin analogue, the second a GLP-1 agonist and also the substance that became clinically known as Ozempic and Wegovy. The research is aimed mainly at obesity and type 2 diabetes. The thinking behind it is simple: two different satiety routes that complement each other.
Two hormones, two routes
The two components each act on a different hormone. Semaglutide mimics the gut hormone GLP-1, which is tied to a feeling of fullness and slower gastric emptying. Cagrilintide mimics amylin, a second satiety route that GLP-1 substances do nothing with. The two together are thought to give a stronger metabolic response in research than either on its own. What is known about this comes from the clinical studies of Novo Nordisk.
REDEFINE and where approval stands
The Phase 3 REDEFINE programme mapped the combination. Substantial weight loss was seen, though the figures came in below what had been hoped for beforehand, and it was precisely that nuance that drew a lot of press attention. As of June 2026 the combination is approved as a medicine nowhere: an application is pending with the US FDA, and a European decision is still awaited. Through the pharmacy, then, it is not to be had. What is known sits in the studies and the medical literature.
Placed next to Wegovy, Ozempic and tirzepatide
The comparison with the well-known weight-loss medicines comes up often, and the difference lies in the number of routes. Wegovy and Ozempic contain only semaglutide and so stay with GLP-1. CagriSema lays the amylin route beside it with cagrilintide. Tirzepatide, sold as Mounjaro and Zepbound, goes about it differently again: one molecule that activates both GLP-1 and GIP. In head-to-head research CagriSema fell just short of tirzepatide, which was widely discussed at the time. One difference weighs more: semaglutide and tirzepatide are approved medicines, this combination is not yet. The individual building blocks sit separately in the range.

In the research the two are studied as one combination, not as two separate injections. That is why they sit pre-mixed in one lyophilised vial with us: the ratio between the components is fixed and you need weigh nothing out in your protocol. What the clinical research describes
The side effects described most often in clinical research sit in the gastrointestinal area: nausea, vomiting and constipation, mainly in the first weeks and when the dose is raised. The profile therefore resembles that of other GLP-1-based substances. Those are observations in humans under medical supervision and so say nothing about laboratory use.
Making it up, storing and a double check
You receive lyophilised powder in one vial in which both peptides have already been brought together. In research that is made up with bacteriostatic water; we give no dosing or administration advice. Keep the unopened vial dry, dark and cool; once the solution is made, it goes into the fridge at 2–8 °C, protected from light, or into the freezer for longer storage.
